Sepsis Alliance Connect Presentation: Preventing Readmissions After Sepsis

When a sepsis patient is discharged after an acute illness, the patient, their loved ones, and their care team might celebrate this important milestone. But according to a 2018 study, 17.5% of sepsis patients are readmitted to the hospital within 30 days of their initial discharge*. What are the most common causes of these readmissions? How can these odds be improved? In this presentation, Dr. Parks Taylor will share how hospitals are working to reduce readmission rates for sepsis patients at the individual and collective level.
